Job summary

Chapters Health System seeks an Advanced Practice Registered Nurse to deliver specialized care for patients with life-changing illnesses in SW Florida. You will provide compassionate nursing care, clinical leadership, and documentation to support quality care and billing integrity.

The role requires Florida APRN licensure, hospice certification preferred, and DEA. You will collaborate with Hope HealthCare Services and participate in broader medical coverage for Pace patients as applicable.

Qualifications

• Must hold an Advanced Nursing Degree, Master’s level from an accredited nursing school.
• Current unrestricted APRN license, Florida.
• Certification in Hospice and Palliative nursing preferred.
• DEA license required.
• Valid Florida driver's license, auto insurance, and reliable transportation.
• Alzheimer’s/Dementia training certification completed.

Responsibilities

• Administer quality nursing care to patients and families.
• Document in patient records to meet medical and billing criteria.
• Provide on-call coverage for patients/families on rotation.
• Coordinate with Hope HealthCare Services and other providers.
