Description

Salary Range: $110,000.00 to $200,000.00 a year with quarterly bonus opportunity.

We have an opportunity for an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) to work in Skilled Nursing Facilities in the Norwalk area. This role offers a flexible work schedule, autonomous work environment, and a pay-for-performance model.

The Nurse Practitioner or Physician Assistant will travel to contracted Skilled Nursing Facilities and will:

• Provide medical care to a complex patient population in short-term rehab and long-term care settings
• Assess patients' medical histories, medication allergies, physical conditions, and examinations to determine appropriate care for acute and chronic conditions
• Order, interpret, and evaluate diagnostic tests to identify and assess clinical problems
• Prescribe medications and treatment regimens as appropriate
• Document findings, care plans, and prognosis in medical records
• Coordinate with care teams to develop comprehensive care plans
• Communicate with patients and families regarding care, discharge, and prognosis
• Follow departmental policies, procedures, and safety standards

Education and Experience

APRN

• Master's or doctorate in nursing
• National certification as a Nurse Practitioner through AANP or ANCC
• Valid Connecticut RN and APRN licenses
• DEA and Connecticut controlled substance licenses
• Experience with elderly patient populations
• Experience with EHR systems

PA

• State license
• Experience (any level)
• Experience with EHR systems
