Job summary

Physicians Alliance of Connecticut (PACT, LLC) is seeking an APRN or PA to provide medical care in Assisted Living Facilities in Farmington and surrounding locations. The role offers autonomy, a flexible schedule, and a pay-for-performance environment.

Providers will travel to contracted facilities, perform assessments for short-term rehab and long-term care, and collaborate with care planning teams to optimize patient outcomes. Connecticut licensure and NP certification are required.

Qualifications

• APRN must hold a master’s or doctorate in nursing.
• Certified Nationally as a Nurse Practitioner (AANP or ANCC).
• CT RN and APRN licenses with NP certification.
• DEA and CT controlled substances license.
• Experience with elderly patient population.
• Experience using EHR systems.

Responsibilities

• Provide medical care to a complex patient population in short-term rehab and long-term care settings.
• Travel to Assisted Living Facilities contracted with PACT, LLC.
• Assess medical history, allergies, and conditions to determine care plan.
• Prescribe medications and treatment regimens as appropriate.

Base Salary: $110,000.00 with quarterly bonus opportunity

An Internal Medical Group has opportunities for Advanced Practice Registered Nurses (APRNs) or Physician Assistants (PAs) to work in an Assisted Living Facility (ALF) in Farmington. The Nurse Practitioner or Physician Assistant will travel to Assisted Living Facilities contracted with PACT, LLC.

• Provide medical care to a complex patient population in the short term rehab and long term care setting


• Analyze patient’s medical history, medication allergies, physical condition and examination to determine best care for acute and chronic medical conditions


• Order, interpret, and evaluate diagnostic tests to identify and assess patient’s clinical problems and health care needs


• Prescribe pharmaceuticals, other medications, and treatment regimens as appropriate for assessed medical conditions


• Record in medical record the physical findings, formulated plans, and prognosis based on patient’s condition within appropriate time frame


• Coordinate with the nursing facility care planning team in the development of comprehensive care plan


• Communicate with patients and family members regarding admission, care plan, discharge needs, prognosis/end of life care as needed


• Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards

Education and Experience

APRN

• The Advanced Practice Registered Nurse is masters or doctorate prepared


• Certified Nationally as a Nurse Practitioner through AANP or ANCC .


• The Nurse Practitioner must have possession of a valid (Connecticut) license as a Registered Nurse and an Advanced Practice Registered Nurse, and be certified as a Nurse Practitioner by an approved certifying body.


• DEA, and Ct controlled substance license
