Job summary

RKM Primary Care in Louisiana seeks a Family Nurse Practitioner to join our ambulatory primary care team. The APRN will provide direct patient care, assess and manage a broad range of health concerns, and work with a community-focused primary care group in an open access, patient-centered environment.

This full-time role (Mon–Fri) offers a salary range of $100,000 to $140,000 per year, with up to 3% annual increases and eligibility for National Health Service Corps loan repayment.

Qualifications

• Must hold active Louisiana APRN license.
• Certified in Family Nurse Practitioner program.
• Ambulatory care experience preferred.
• Current Basic Life Support (BLS) certification.

Responsibilities

• Assess, plan, intervene and evaluate patients.
• Provide health education to patients and build long-term relationships.
• Follow guidelines for procedures and pharmacological management.
• Order referrals for on-site and off-site services and monitor completion.

APRN, Family Nurse Practitioner

Join RKM as a Family Nurse Practitioner and make a difference in the community by providing primary care in an ambulatory health setting as part of a primary care team. Since 1999, our mission is to provide open access to quality patient centered healthcare in the communities in which we live. RKM Primary Care is a Federally Qualified Health Center with 10 clinics in 5 parishes.

Job Overview

Our sites qualify for the National Health Service Corps loan repayment program (individual qualifications apply)

Direct Patient Care

• Assessment, planning, intervention and evaluation of patients
• Provide health education to patients while building and maintaining a long-term patient base
• Follow practice guidelines for procedures and in pharmacological management
• Order referrals for on-site services and off-site specialist and monitor completion for total patient care

Quality Management

Meets performance standards set by the organization and supervises patient care team’s performance. Importance and impact of documentation in the EHR

Supervision

• Provide direct supervision over the primary care team that usually consists of a nurse and a medical assistant.
• Work with Clinic Practice Manager with administrative duties related to your team
• Communicate clearly with the Primary Care Team, Assistant Clinical Director and Chief Clinical Officer
• Assist with team coverage through the organization as needed/requested

Qualifications / Skills

• Advanced Practice Registered Nurse currently licensed to practice in the State of Louisiana
• Certification from a formal recognized certifying Family Nurse Practitioner Program
• Prefer experience in ambulatory care setting
• Current Basic Life Support
