Advocate Health’s outpatient cardiology clinic at Imani Village in Chicago is seeking an RN/APRN to join a collaborative team delivering evidence-based cardiac care in an outpatient setting.

You’ll manage 14–16 follow-up patients daily, perform chart reviews, educate patients, and document in the EMR, all with a predictable 8:00 AM–4:30 PM schedule, no nights, weekends, or holidays.

Formación

• Active RN and APRN license in state of practice.
• National board certification in clinical practice area.
• DEA Registration active prior to hire.
• Illinois Controlled Substance license active.
• BLS and/or ACLS certification within 6 months of hire.
• Master’s Degree in Nursing.
• No prior experience required.

What You’ll Do

• - Manage 14–16 follow-up patients daily in an outpatient cardiology setting
• - Perform chart reviews and in-basket management
• - Deliver evidence-based care and patient education

Clinical Responsibilities

• - Assess, diagnose, and manage treatment plans appropriate for patient age, acuity, and condition.
• - Order, perform, and interpret diagnostic studies; prescribe pharmacologic and non-pharmacologic therapies.
• - Manage conditions using evidence-based care, cost-effectiveness, and risk-benefit analysis.
• - Provide health promotion, disease prevention, and patient/family education.
• - Collaborate within an interdisciplinary team; escalte care when necessary.
• - Maintain accurate, timely documentation in the EMR; ensure compliant coding and billing.
• - Facilitate coordinated care and clear communication among healthcare team members.
• - Perform office procedures within granted privileges and competency.

• - Maintain and advance clinical skills through ongoing education.

Minimum Education and Experience

• - Licensure: Active RN and APRN license(s) in state(s) of practice
• - Certification: National board certification in clinical practice area
• - DEA Registration: Active prior to hire
• - Illinois Practice: Active Illinois Controlled Substance License
• - Certifications: BLS and/or ACLS (within 6 months of hire)
• - Education: Master’s Degree in Nursing
• - Experience: No prior experience required

• - High level of clinical proficiency and decision-making skills.
• - Ability to work independently and collaboratively.
• - Proficiency with EMR and evolving technology.
• - Excellent communication and organizational skills.

Physical Requirements
