Job summary

ClareMedica Health Partners LLC seeks a licensed Nurse Practitioner to manage health problems and coordinate care for clinic patients in accordance with state and federal rules and nursing standards. The NP will perform age-appropriate history and physical exams, order tests, and develop and implement treatment plans.

Qualifications

• Licensed ARNP in New York state.
• Graduate of an accredited Nurse Practitioner program.
• Active NP license.
• Family Medicine NP certification preferred.
• BLS/ACLS certifications preferred.
• Strong computer and data-management skills.

Responsibilities

• Perform age-appropriate history and physical exams.
• Order and interpret diagnostic tests as needed.
• Prescribe pharmacologic and non-pharmacologic treatments.
• Implement interventions to maintain physiologic stability.

ESSENTIAL FUNCTIONS:The Nurse Practitioner is a licensed independent practitioner who is responsible for managing health problems and coordinating health care for clinic patients in accordance with State and Federal rules and regulations and the nursing standards of care (assessment of health status, diagnosis, development of plan of care and treatment, implementation of treatment plan, and evaluation of patient status).
