Position: APRN Team Manager
Departments: UPAC, UHealth SoLé Mia

The APRN Team Manager functions as a clinical leader and team member, delivering care to a patient panel, and overseeing the clinical practice of an inter-professional team that may include nurses, medical assistants, and others. The role involves planning, implementation, coordination, and evaluation of care outcomes, working in partnership with the patient care team to ensure effective care delivery and achievement of organizational goals.

Key Responsibilities:
  1. Develop, lead, and manage clinical practice, quality, and safety of patient care within an interdisciplinary team.
  2. Coordinate care with various providers, including licensed independent practitioners, care managers, and others.
  3. Participate in hiring, supervision, and performance evaluations of team members.
  4. Identify opportunities for interdisciplinary collaboration to meet team and patient goals.
  5. Utilize resources to improve practice and patient outcomes, especially for high-risk patients.
  6. Disseminate clinical knowledge through publications and professional meetings.
  7. Represent the department on committees and task forces.
  8. Collaborate with physicians and team members to refine care models and analyze care results.
  9. Foster effective interpersonal relationships and provide direct patient care, counseling, and teaching.
  10. Perform physical exams, order and interpret tests, and develop follow-up care plans.
  11. Prioritize patient management efficiently, incorporating evidence-based practices.
  12. Promote health maintenance, prevention, and patient education.
  13. Engage in quality improvement activities and utilize outcome data for ongoing enhancements.
  14. Develop and assess internal controls and ensure compliance with policies.
