Job Summary

The Nurse Practitioner (NP) provides medical management to patients on the inpatient rehabilitation unit at South University Hospital. Obtains a thorough medical history from patients and, depending on the patient population and setting, will perform an examination and/or assessment to determine patients' needs. Collaborates with other healthcare professionals to analyze and interpret information collected from patients, medical records, symptoms, physical findings, or diagnostic information, to develop and establish appropriate diagnoses. Rounds on patients to demonstrate the importance of care coordination and navigation. Works in collaboration with providers and independently to provide care. Formulates a treatment plan for patients and prescribes medications based on efficacy, safety, and cost as legally authorized. Maintains complete and detailed records of patients' health care plans and prognoses in a timely manner. Makes necessary healthcare referrals. Provides patients with information needed to promote health, reduce risk factors, or prevent disease or disability. Counsels patients and family members about self-management and treatment plans for health issues, tailoring instructions to patients' individual circumstances. May participate in clinical research studies. Incorporates evidence-based practice guidelines into care. Maintains current knowledge of state legal regulations for advanced practice provider practices, including reimbursement of services.

Qualifications

  1. Completion of a master’s, postmaster’s, or doctorate from a nurse practitioner program accredited by the Commission on the Collegiate of Nursing Education or National League for Nursing Accrediting Commission.
  2. Demonstrated current competence and provision of care, treatment, or services for an adequate volume of patients in the past twelve months, or completion of master’s/post-master’s degree program in the past twelve months.
  3. Current licensure by the applicable state board of nursing for advanced practice.
  4. Current Drug Enforcement Administration (DEA) permit to prescribe controlled substances.
  5. Certified Adult Gerontology Nurse Practitioner (AGPCNP-BC) or (A-GNP) or (AGACNP-BC) or (ACNPC-AG) or Certified Family Nurse Practitioner (NP-C) or (FNP-BC).
  6. Obtains and subsequently maintains required department-specific competencies and certifications.

Schedule M-F, inpatient setting, Hospital (no clinic). Experience preferred. New grads may be considered; CVs should reflect the ability to treat patients in complex settings with many medical issues that may challenge new grads. MD supporting at all times.

Duties:

  1. Assist in the management of hospital care for rehab patients.
  2. Responsible for diagnosis and treatment of complex medical patients, in conjunction with the interdisciplinary care team.
  3. Works in an interdisciplinary team to provide the highest quality of patient care.
  4. Complete medical progress notes daily for the patients assigned to their care.

Team Composition: The Hospitalist team consists of approximately 60 providers: 55 physicians and 5 APPs. The Rehab team will consist of a Hospitalist physician, this APP, Rehab physicians, APPs, and nursing staff.
