Job summary

Sanford Health in Canby, MN is seeking a Family Medicine Nurse Practitioner to provide advanced nursing care across all ages, from infants to seniors. You will assess, diagnose, treat, and manage acute and chronic illnesses and collaborate with physicians and the healthcare team.

Responsibilities include exams, health assessments, prescribing medications, care planning, and patient education, with on-call coverage outside regular hours.

Qualifications

  • Completion of a master’s, postmaster’s, or doctorate from an accredited nurse practitioner program accredited by the Commission on the Collegiate of Nursing Education or National League for Nursing Accrediting Commission.
  • If Licensing prior to August 1, 1995, master's degree in nursing is preferred.
  • Experience in primary care or family medicine preferred
  • Current licensure as a Registered Nurse and Nurse Practitioner in each state of practice.
  • Current Drug Enforcement Administration (DEA) permit to prescribe controlled substances in each state of practice.
  • State Controlled Substance License (if required by State)
  • National board certification as a Family (FNP-C) or (FNP-BC)
  • Obtains and subsequently maintains required specific competencies and certifications as outlined on privilege form.

  • Coordinate referrals to specialists when needed.
  • Perform minor procedures (e.g., wound care, suturing) as appropriate.
  • Document patient care accurately in the EHR system.
  • Participate in quality improvement and practice initiatives.
  • Collaborate effectively with physicians, nurses, and other healthcare team members.

Job Summary
