Sanford Health, Marshfield Clinic Menomonie Center, seeks a Family Medicine Nurse Practitioner to provide comprehensive primary care for patients of all ages. You will assess, diagnose, treat, and manage acute and chronic conditions in collaboration with physicians and other providers.

Responsibilities include physical exams, health assessments, prescribing medications, creating care plans, patient education, and coordinating referrals as part of a supportive clinical team.

Qualifications

• Master’s or Doctorate degree from an accredited Nurse Practitioner program.
• If APRN licensed before Aug 1, 1995, master's in nursing preferred.
• National board certification as a Family Nurse Practitioner (FNP-C or FNP-BC).
• Maintains required competencies and certifications per privilege form.

Department Details

Join a busy primary care practice with an established patient panel and significant opportunity for growth in the Menomonie market. This position offers the opportunity to help rebuild and shape the future of the practice following recent provider transitions. The incoming provider will have the ability to develop a strong practice from the ground up while caring for an existing patient population with continued demand for primary care services. The department also includes newer team members, creating an opportunity to help build the culture, workflows, and long-term direction of the practice.

Job Summary

The Family Medicine Nurse Practitioner provides advanced nursing care to patients of all ages, from infants to older adults. This role involves assessing, diagnosing, treating, and managing acute and chronic illnesses, promoting health and wellness, and working in collaboration with physicians and other healthcare providers. Conduct comprehensive physical exams and health assessments

Obtain and interpret patient histories and diagnostic tests

Diagnose and treat common acute and chronic medical conditions

Prescribe medications and therapeutic treatments within the scope of practice

Develop and implement individualized care plans

Provide health education and counseling on disease prevention and healthy lifestyle choices

Manage follow-up care and coordinate referrals to specialists when needed

Perform minor procedures as appropriate (e.g., wound care, suturing)

Document patient care accurately in the electronic health record system

Participate in quality improvement and practice initiatives

Collaborate effectively with physicians, nurses, and other healthcare team members

Maintain current knowledge of evidence-based practice guidelines.

Qualifications

Master’s or Doctorate degree from an accredited Nurse Practitioner program.

If APRN was licensed prior to August 1, 1995, master's degree in nursing is preferred.

Experience in primary care or family medicine preferred

Current licensure as a Registered Nurse and Nurse Practitioner in each state of practice.

Current Drug Enforcement Administration (DEA) permit to prescribe controlled substances in each state of practice.

State Controlled Substance License (if required by State)

National board certification as a Family (FNP-C) or (FNP-BC)

Obtains and subsequently maintains required specific competencies and certifications as outlined on privilege form.
