Job summary

Trinity Health Medical Group in Grand Rapids, Michigan, is seeking a passionate Physician Assistant or Nurse Practitioner to join the Primary Care & Pediatrics - Cherry team. This full-time, 1.0 FTE position offers autonomy within a supportive, mission-driven environment.

Office hours are Mon 7am–6:30pm, Tue 7am–6pm, Wed-Fri 7am–5pm, with a location at 245 Cherry St SE. You will provide direct care across all ages, collaborate with physicians and APPs, document in Epic EMR, and participate in

Qualifications

• Provider experience required.
• Board certified prior to employment.
• Active Michigan medical license (or ability to obtain).
• DEA license.

Responsibilities

• Providing direct patient care.
• Collaborating with multidisciplinary teams.
• Participating in teaching and mentoring.
• Documenting in Epic EMR.
• Engaging in quality improvement initiatives.
• Supporting community health efforts.

• Office Hours:
  • Monday 7am to 6:30pm
  • Tuesday 7am to 6pm
  • Wednesday through Friday 7am to 5pm
• Location: 245 Cherry St SE Grand Rapids, MI 49503
• Specialty:Primary Care - Family Medicine, must be able to see all ages
• Clinical Team Size:4 APPs, 6 physicians, and 2 pediatricians. Office total 46
• Call Requirements:No call requirements for an APP
• Additional details: Looking for an experienced APP to partner with our office
