Qualifications

• Bachelor's Degree in Pharmacy is required.
• Doctorate in Pharmacy preferred.
• 1 year PGY1 Pharmacy Residency required.
• 3 years in area of specialization required.
• 1 year PGY-2 Pharmacy residency preferred.
• Registered Pharmacist (RPh) license required.
• Board Certification required within 24 months if applicable.
• Basic Life Support (BLS) certification required.

Responsibilities

• Consult with healthcare professionals on safe and appropriate medication use.
• Lead ongoing staff education for clinical staff pharmacists.
• Direct and precept students and residents.
• Review adverse events and drive improvements in the medication use process.
• Review prescriptions for appropriateness and provide safety information.
• Participate in local and system-wide committees to improve practice.
• Provide clinical pharmacy services per hospital policies (pharmacokinetics, dosing, antimicrobial stewardship, transitions of care).
