Job summary
Franciscan Health Lafayette Central is seeking a Clinical Pharmacy Specialist to provide comprehensive pharmacotherapy services and lead education for clinical staff pharmacists, students, residents, and other healthcare professionals.
no weekends and no holidays; days vary Monday - Friday 8:00 AM – 4:30 PM. Expect collaboration, formulary leadership, and ongoing reviews of medication use processes in a hospital setting.
Qualifications
- Registered Pharmacist (RPh) licensed by State Licensing Board.
- Board Certified in applicable area of specialty or pharmacotherapy within 24 months.
- BLS certification as required by unit.
Responsibilities
- Collaborate with allied health disciplines to provide quality patient care.
- Consult with Healthcare professionals and patients on medications selection and use.
- Lead ongoing staff education for clinical staff pharmacists and residents.
- Direct and precept students and residents.
- Lead cost-effective formulary management initiatives.
- Review adverse medication events and propose process improvements.
- Review literature and recommend improvements to medication use.
- Participate in local and system-wide committees to improve medication use.
- Draft and revise medication-related policies and protocols.
- Provide clinical pharmacy services per hospital policies (pharmacokinetic consults, dosing, nutrition, anticoagulation, antimicrobial stewardship, transitions of care).
