Qualifications

Responsibilities

• Provide total comprehensive nursing care based on the plan of care.
• Assess patient and family needs across physical, psychological and socioeconomic domains.
• Educate patients and families regarding care, treatment, and discharge planning.
• Identify gaps in care and communicate with nursing staff and providers.
• Follow up with patients after discharge and support PFCC initiatives.
