The role requires a Doctor of Pharmacy, ASHP PGY1/PGY2, VT license or eligibility, BLS, Teaching Certificate, and BCACP/CSP eligibility, with Epic experience and a focus on improving access and outcomes.

Qualifications

• Doctor of Pharmacy degree and ASHP‑accredited PGY1/PGY2 residency.
• VT license or eligibility to obtain license.
• BLS certification and Teaching Certificate.
• Eligible to sit for BCACP or CSP board certification.
• CITI training completed.
• Experience with Epic; insurance access and appeals; educate staff; quality improvement.

Responsibilities

• Manage the specialty pharmacy patient management program for gastroenterology and hepatology.
• Oversee coordination with gastroenterology and hepatology teams.
• Navigate insurance access issues for specialty meds and infusions.
• Educate pharmacy technicians, pharmacists, providers, and nurses; support quality improvement projects.
