Qualifications

  • Experience in customer service.
  • Preferred clinical and/or medical social work experience.
  • Preferred experience in group work, case work, and community methodologies.

Responsibilities

  • Participates in patient admission process and serves as hospital representative and patient advocate.
  • Coordinates and facilitates services for patients and families throughout the Stormont Vail Health system.
  • Performs social work assessments considering bio-psychosocial, cultural, developmental and spiritual aspects of illness.
  • Develops case management/treatment plans based on assessment and multidisciplinary input.
  • Addresses patient/family understanding and adjustment to illness to maximize benefits of intervention.
  • Initiates discharge planning and coordinates with internal and external teams.
  • Develops and facilitates groups for patients and significant others as needed.
  • Maintains confidentiality and adheres to professional ethics; participates in patient care conferences.
  • Provides patient education regarding diagnosis and treatment; offers crisis and brief therapy.
