The University of Wisconsin Medical Foundation in Madison seeks an Ambulatory Social Work Specialist. This 100% FTE day-shift role runs Monday through Friday, 8:30 am–5:00 pm, at the Administrative Office Building in Middleton, WI.
You will triage ambulatory clinic social service needs, delegate cases, document and report, coordinate care, and facilitate community referrals while applying Trauma-Informed principles to improve adherence and patient wellbeing.
