Job summary

The University of Wisconsin Medical Foundation in Madison seeks an Ambulatory Social Work Specialist. This 100% FTE day-shift role runs Monday through Friday, 8:30 am–5:00 pm, at the Administrative Office Building in Middleton, WI.

You will triage ambulatory clinic social service needs, delegate cases, document and report, coordinate care, and facilitate community referrals while applying Trauma-Informed principles to improve adherence and patient wellbeing.

Qualifications

• Bachelor's Degree in psychology, social work, counseling, or related behavioral science field is required.
• 1 year related experience is required.
• Experience with diverse populations with complex social/emotional needs is required.
• Previous medical setting experience is preferred.

Responsibilities
