Description

The Senior Clinical Pharmacist will collaborate with physicians and staff to provide coordinated, comprehensive, high-quality care to patients. The responsibilities of the clinical pharmacist will predominantly fall into the following areas: (1) Patient Care - The Senior Clinical Pharmacist is responsible for providing collaborative practice services in collaboration with the healthcare team. (2) Population Management - The Senior Clinical Pharmacist may utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization.

Required Qualifications

• Bachelor's Degree in PHARMACY from an accredited School of Pharmacy or

• PharmD in PHARMACY from an accredited School of Pharmacy.

• Coursework/Training: Postgraduate Year One (PGY-1) Pharmacy Residency or equivalent education/experience.

• Upon hire: Washington Pharmacist License.

• Within 2 years of hire: Certification from Board of Pharmacy Specialties.

• Upon hire: Board of Pharmacy Preceptor License - when eligible and where applicable.
