Job summary

Parrish Medical Group in Titusville, FL is seeking a full-time Registered Nurse to join an interprofessional team. The RN will coordinate patient care, perform clinical and procedural support within scope, and ensure documentation and patient education are completed in the EMR.

Responsibilities include triage, data collection, care plan execution, and collaboration with providers to meet patient needs across settings. Strong licensing and BLS are required.

Qualifications

• Graduate of an RN education program; Associate Degree required; Bachelor Degree in Nursing preferred.
• Active and clear State of Florida RN license required.
• BLS through the American Heart Association (AHA) required.
• 6 months to < 1 year RN experience; ambulatory/procedural setting preferred.
• Strong collaboration and patient-centered care across interprofessional teams.

Responsibilities

General Description:

Under the direction of the Practice Manager, this position is responsible for working collaboratively as an active member of the inter professional team to provide therapeutic patient and family centered care. The Registered Nurse continuously collects and assesses data related to patient health status either via in-person visit or to include phone triage to develop, execute and evaluate the plan of care with the support of guidelines set by the rendering physician. This includes assisting with clinical and/or procedure needs within allotted scope of practice. The Registered Nurse establishes goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers. The Registered Nurse supervises care in accordance with policies and scope of practice set forth by Parrish Healthcare and the State of Florida/applicable authority entity. Will participate in continuous quality improvement initiatives and assumes responsibility for their professional development. This position shall exemplify the desired Culture of Choice and philosophies of Parrish Healthcare.

Key Responsibilities

1. Executes nursing interventions by collaborating with the inter professional team incorporating appropriate standards of care, practice and patient outcomes


2. Assesses and systemically collects data related to patient health status and works in collaboration with the care team to develop the plan of care


3. Evaluates patient's response to set plan of care, to include obtaining necessary order sets, authorization and/or any additional care plan need to ensure patient completes and or has full plan of care established and completed.


4. Utilizes data to continuously meet care coordination needs across settings in conjunction with the patient, family and/or other care providers to include assuming responsibility and accountability for effectively managing nursing care of individual patients


5. Documents nursing assessment and interventions to reflect comprehensive and integrated approach to patient care in which should include providing care that is safe, ethical, patient- and family-centered, culturally congruent and evidence-based within their scope of practice set guidelines


6. There are skills and responsibilities that are common across the ambulatory setting (primary or specialty care). RN-Ambulatory may perform any or all of these skills and responsibilities. These may include but are not limited to:
   
   
   
   • Telephone or in-person triage by providing patient education and assessment feedback
   
   
   • Review test results and provide bucket management for assigned providers and own bucket management
   
   
   • Review and manage communications from patients via received portal messages or voicemails
   
   
   • Assist by following sterile technique processes during support to procedural tasks while providing urgent and emergent patient care as warranted
   
   
   • Assist or perform procedures (e.g. ECGs, dressing changes, subcutaneous or intra-muscular injections, etc.) via provider support and/or via own schedule in clinic setting management
   
   
   • Assist with medication management to include refill requests, injections in the office or as applicable
   
   
   • Complete accurate and timely documentation in the electronic medical record
   
   
   • May float to other clinics on an as needed for operations need
   
   
   
   


7. Work independently within their scope of practice to achieve clinical outcomes


8. Maintain a level of professional development through continuing education, quality improvement initiatives and sharing of knowledge


9. Leadership through collaborative partnerships with care partners, patients or visitors

11. Performs similar or related duties as assigned.


12. Knows fire, disaster and safety procedures and regulations as it pertains to the work area.

Requirements

Formal Education

• Graduate of an RN education program (accredited preferred); Associate Degree in Nursing required. Bachelor Degree in Nursing preferred

Work experience

• 6 months to < 1 year. Preferred relevant RN experience in procedural our ambulatory setting.

Required Licenses, Certifications, Registrations

• Active and clear State of Florida RN license required.


• BLS through the American Heart Association (AHA) required.
