Must have within 90 days of hire.Certified Wound Ostomy Nurse - WOCNCB - Magnet PreferredCertified Wound Care Associate - AAWM - Magnet PreferredWhat you will doProvides case management to a group of specified patients providing specific assessments according to wound care clinical practice guidelines and policies and procedures, complete all required related documentation, including review of appropriate I-Heal reports and acts as patient advocate in delivery and coordination of patient careFormulates a prioritized and goal directed plan of care, which is based on patient’s presenting problems and projected outcomes.Provides direct patient care as defined within the scope of the state’s nurse practice act. Performs assessment/data collection in an ongoing and systematic manner, focusing on physiological and cognitive status. Administers prescribed medications, infusions, and/or treatments in accordance with nursing standards.Functions to establish priorities of patient care based on essential patient needs and available unit resources of time, personnel, equipment and supplies. Prepares equipment and aids provider during treatment, examination and testing of patients.Evaluates the effectiveness of the health care process (self, systems, environment, and instrumentation) in meeting outcomes. Observes, records, and reports patient’s condition and reaction to drugs and treatments to provider. Provides medication as directed.Documents in patient and department records in a timely, accurate and concise manner according to department standards.
