Job summary

Duke University Hospital in Durham, NC seeks a clinical pharmacist to deliver ambulatory pulmonary services both in clinic and via telehealth. You will support therapy plans, patient education, and laboratory monitoring while collaborating with Duke Health teams to ensure safe, effective medication use.

Licensure as a pharmacist in NC is required; a Doctor of Pharmacy degree is expected, with residency or ambulatory care experience preferred.

Qualifications

• Pharmacist licensure or eligibility to practice in North Carolina.
• BS Pharmacy degree or Doctor of Pharmacy degree required; advanced certifications preferred.
• PGY1/Residency or ambulatory care experience preferred but not required with a PharmD.

Responsibilities

• Provide ambulatory pulmonary clinical pharmacy services to patients in clinic and via virtual care.
• Develop therapy plans, monitor labs, and educate patients and staff on medications.
• Coordinate with Duke Health pharmacies and care teams to ensure access and adherence.
• Participate in quality improvement, outcomes tracking, and guideline development.
