Qualifications

• Doctor of Pharmacy degree or equivalent with advanced clinical training.
• PGY1 Pharmacy Practice Residency and PGY2 Ambulatory Care/Primary Care Residency.
• Board certified ambulatory care pharmacist (BCACP) or eligible to sit.
• Ability to lead work groups and communicate effectively.

Responsibilities
