Job summary

Qualifications

• Current Pharmacist’s license issued by the Oregon State Board of Pharmacy.
• BS in Pharmacy or Pharm.D. plus advanced training or residency preferred.
• Ability to work independently with strong clinical judgement.

Responsibilities

• Identify, resolve and document complex therapeutic problems for diverse patient populations.
• Interpret prescriber orders for accuracy and appropriateness; document discrepancies.
• Coordinate discharge medications and educate patients about home regimens.
• Monitor drug therapy and report adverse effects; consult with providers as needed.
• Respond to drug information requests and apply current literature to patient care.
• Develop treatment guidelines and participate in education programs with peers.
• Supervise technicians, students, interns, and residents; monitor performance.
