Job summary

UVM Health - UVM Medical Center is seeking an experienced ambulatory pharmacist clinician to optimize drug use in high-risk populations and contribute to ambulatory care teams across rheumatology, neurology, infectious disease, oncology, nephrology, and dermatology.

The role focuses on direct patient care, teaching, consultation, policy development, and supporting pharmacy operations including prior authorization and medication therapy outcomes.

Qualifications

• Doctor of Pharmacy degree required with ASHP-accredited residency or equivalent experience.
• Licensed pharmacist in the State of Vermont.
• 3 years of advanced practice experience in ambulatory care or clinical outpatient setting preferred.

Responsibilities

• Provide direct patient care and clinical pharmacotherapy expertise.
