Position Details

Job: Ambulatory Pharmacist Clinician

Hours: Mon, Tues, Fri: 8:00am-4:30pm; Thu: Evening STI Clinic 4:00pm-8:00pm (LGHP Family Medicine Twin Rose) (28 hours/week)

Location: Various locations in Lancaster area

Department: Pharmacy and IV Solutions

Responsibilities
  • Provide clinical pharmacy services including pharmacist-managed clinics, transitions of care communication, and other direct patient care activities. Design, recommend, monitor, and evaluate patient-specific pharmacotherapy. Serve as a clinical pharmacy expert for ambulatory services, with responsibilities in training, education, competency assessment, and development of clinical programs.
Minimum Qualifications
  • Pharm.D. and ASHP-accredited Pharmacy Practice Residency or equivalent experience.
  • ASHP-accredited Specialty Residency or equivalent preferred.
  • Current licensure or eligibility to reciprocate in Pennsylvania.
  • Board Certification may be preferred depending on specialty.
  • At least one year of ambulatory practice experience.
  • Good standing with Medicare, Medicaid, and other federal/state health insurance programs.
  • Three years of ambulatory practice experience preferred.
