Qualifications

• PharmD with PGY-1 residency or 5 years pediatric hospital experience.
• PGY-2 specialty residency in pediatrics and/or Hematology-Oncology preferred.
• Ambulatory oncology experience preferred and hospital pharmacy background.
• Active Pharmacist License in DE/NJ/PA required; BCOP/BCPPS within 12 months.

Responsibilities

• Review medication orders for pediatric hematology/oncology patients and assess appropriateness.
• Monitor and adjust chemotherapy and supportive care regimens.
• Counsel patients and families on medications, including oral chemotherapy and adherence.
• Collaborate with multidisciplinary team and Specialty Pharmacy for access and continuity of care.
• Develop guidelines, pathways, and EMR-based decision support.
• Participate in clinical research protocols and quality improvement projects.
• Precept students and residents; contribute to education within Pharmacy Services.
