Position Overview

This position requires travel to our central region clinics and candidates without a BSN must obtain it within 2 years of hire.

Description

The Ambulatory Nurse Care Coordinator supports high-quality patient care through education and care management, acting as a liaison among patients, physicians, and staff. The role aligns with NM's mission, values, and compliance standards.

Responsibilities
  1. Advocate for patients and assist in navigating care settings.
  2. Manage care coordination for high and moderate-risk patients, including new and low-risk patients.
  3. Review patient data to identify care needs.
  4. Assess and address barriers to effective care.
  5. Collaborate with multidisciplinary teams to develop and evaluate care plans.
  6. Provide triage and educational support to patients and families.
  7. Coordinate post-discharge care, referrals, and community resources.
  8. Manage high-risk cases to reduce readmissions and costs.
  9. Develop and support patient-centered care plans.
  10. Maintain and review patient health information.
  11. Monitor care quality and satisfaction, participating in quality improvement initiatives.
  12. Provide after-hours on-call support.
  13. Perform other duties as required.
Qualifications
Required:
  • Current Illinois RN license.
  • BSN or willingness to obtain within 2 years.
  • 3-5 years of clinical experience, with 5 preferred, in chronic conditions, care coordination, and related areas.
  • Willingness to travel.
Preferred:
  • 3 years in care management or coordination.
  • Case Management certification or willingness to obtain.
  • Experience in ambulatory nursing and process improvement.
