Job summary

Live Well Homecare is seeking a full-time Ambulatory Nurse to provide direct patient care in a clinic setting, collaborating with providers to create and implement individualized care plans and to assess physical, psychosocial, and emotional needs.

You will support patient management, document assessments accurately, educate patients using appropriate tools, and uphold safety and compliance with clinical guidelines and documentation standards.

Qualifications

• RN licensure and BLS certification.
• Bachelor’s degree preferred.
• 2-5 years of nursing experience; 5-10 years preferred.
