Job summary

Caring Health Center, Inc. is seeking a Nurse Practitioner to provide comprehensive care on-site in Springfield, MA. The NP will assess, diagnose, and manage acute and chronic conditions, with a focus on family planning and preventive care.

Requirements include MA NP licensure and DEA; certification in Family Practice, Adult, or Pediatric; and ability to work with a multidisciplinary team. Bilingual candidates are encouraged to apply.

Qualifications

• Massachusetts NP licensure and/or eligibility for certification.
• Certification in Family Practice, Adult, or Pediatric.
• DEA licenses.
• Ability to collaborate with multi-disciplinary team.
• Experience in ambulatory care and/or public health preferred.
• Fluency in Spanish, Arabic, Russian, Nepali, Vietnamese or another language served at CHC.
• Special expertise in care for underserved, culturally diverse populations.

Responsibilities

• Conduct comprehensive patient assessments.
• Identify potential and actual health problems.
• Evaluate patient responses.
• Diagnose acute and chronic illness and develop treatment plans.
• Involve patient and family in treatment decisions and wellness goals.
• Ensure chart accuracy and timely documentation (same day to 72 hours).
• Collaborate with CHC physicians and other providers.
• Refer patients to specialists when needed.
• Maintain CME/CEU and licensing requirements.
