Job summary

Chesapeake Regional Healthcare is seeking a Licensed Practical Nurse (LPN) to support assessment, planning, implementation, and evaluation of patient care under supervision. You will document clinical information, obtain vitals, and assist with education and care coordination across the healthcare team.

The role requires graduation from an accredited Practical Nursing School, licensure in Virginia or North Carolina, and BLS certification on hire.

Responsibilities

• Assist with patient education and document clinical information in the medical record.
• Pre-assess patients by reviewing history and collecting current information.
• Obtain vitals for each patient as applicable.
• Chaperone patient encounters with physicians.
• Prepare room and patient for provider encounter.
• Provide interventions ordered by the provider (medications, dressings, IVs, etc.).
• Monitor and record patient responses and status.
• Coordinate referrals, homecare, and therapy as ordered.
