Job summary

Baldwin Family Health Care seeks a Clinical Pharmacist to join our ambulatory care team. The role focuses on direct patient care, chronic disease management, and collaboration with providers to optimize therapy and outcomes.

The successful candidate will serve as primary preceptor for the PGY1 Community-Based Pharmacy Residency Program and act as Residency Program Coordinator, fostering teaching and residency development in an inclusive, rural health setting.

Qualifications

• PharmD with active Michigan license or eligibility.
• Experience in outpatient or ambulatory care settings preferred.
• Ability to work in a multidisciplinary team and educate patients.

Responsibilities

• Deliver evidence-based, cost-effective medication management.
• Conduct comprehensive medication reviews and reconciliation.
• Provide patient education on medications and disease states.
• Monitor outcomes and manage drug-related problems.
• Precept PGY1 residents and lead residency activities.
