Qualifications

• US Citizenship required.
• Registered Nurse (RN) with active license.

Responsibilities

• Performs independent telephone and clinical triage and coordinates care through a multidisciplinary team.
• Establishes priorities for patient care and monitors progress toward wellness.
• Educates patients as an integral part of care and supports self-care.
• Triages walk-in patients to assess severity and need for provider intervention.
• Documents events and communicates with providers about determinations and situations.
• Assists with examinations and procedures and educates patients on high-risk or stressful procedures.
