Job description

Position Title: Ambulatory Clinic RN Care Coordinator - ENT Medical Pavilion

Position Summary / Career Interest: The Ambulatory Clinic RN Care Coordinator is a licensed registered professional nurse who is a clinical expert providing ethical, evidence-based, safe, and compassionate nursing care. The focus is on patient safety and quality of nursing care by identifying and clarifying patient needs, performing clinic-related patient care, conducting health education, promoting patient advocacy, coordinating healthcare services, and evaluating patient outcomes. The RN Care Coordinator will support assigned physicians by communicating with the patient/family/caregiver and relaying information to the physician. They will provide support and nursing care services during the physician's clinic sessions and ensure the patient/family/caregiver understands the plan of care. They will verify all orders placed during the clinic session have been completed and the physician has been notified of the results.

Responsibilities and Essential Job Functions:

  • Responsible for management of patient flow, triage, and direct patient care.
  • Liaison of communication between front desk staff, nursing staff, physicians, and clinic manager.
  • Manage the care of the patient population by developing expected patient outcomes, defining appropriate resource utilization, and working with the provider to set patient goals.
  • Provide nursing support/coordination to optimize patient outcomes and improve patient quality of life through clinical and educational programs and/or support services.
  • Support patient self-management of disease.
  • Coordinate continuity of patient care with patients and families following hospital admission, discharge, and ER visits.
  • Manage high-risk patient care, including patients with multiple co-morbidities or high risk for readmission, including a registry.
  • Develop relationships with patients and competently use assessment skills to identify physical, psychosocial, financial, and environmental health care needs.
  • Educate the patient and appropriate ancillary personnel about health care needs, safety issues, and benefits including patient education.
  • Utilize available electronic tools, such as EMR, to efficiently document assessments, clinical findings, plans of treatment, and progress toward goals.
  • Observe, record, and report patient's condition and reaction to drugs and treatments to physicians. Educate patient/family about diagnostic procedures, medications, nutrition, and maintenance of health and wellness.
  • Maintain/review patient records, charts, and other pertinent information. Record tests and examination results and follow up with patients.
  • Respond to/refers incoming patient treatment-related phone calls. Instruct patients and families regarding medications and treatment instructions.
  • Maintain patient confidentiality.
  • Document using EMR to maintain and/or develop patient records in compliance with practice standards.
  • Perform the professional, clinical, and/or technical competencies of the assigned unit or department.

Required Education and Experience:

  • High School Graduate
  • Bachelor Degree in Nursing
  • 2 or more years of RN experience

Preferred Education and Experience:

  • Previous medical office experience

Required Licensure and Certification:

  • Basic Life Support Heartsaver (BLS HS) (Non-Clinical) - American Heart Association (AHA) or obtained within 14 days.
  • Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing.

Preferred Licensure and Certification:

  • Must have current active multi-state Registered Nurse (RN) license in good standing in the state in which you live OR active single state RN license in good standing in the state you will be working, with proof of application for a multi-state RN license in the state in which you live.
    Staff hired prior to 2/1/2024 are required to obtain a multi-state RN license from the state in which they live at the time of their next RN license renewal by 3/31/2026.

Knowledge Requirements:

  • Basic typing and word processing ability.
  • Excellent analytical, critical thinking, and problem-solving skills.
