Position Summary / Career Interest:
The Ambulatory Clinic RN Care Coordinator is a licensed registered nurse who provides ethical, evidence-based, safe, and compassionate nursing care. The role focuses on patient safety and quality of care by identifying patient needs, performing clinic-related care, conducting health education, promoting advocacy, coordinating healthcare services, and evaluating outcomes. The RN Care Coordinator supports physicians by communicating with patients and families, relaying information, and ensuring understanding of the care plan. They verify all orders during clinic sessions are completed and communicate results to physicians.
Responsibilities and Essential Job Functions
- Manage patient flow, triage, and direct patient care.
- Serve as liaison among front desk staff, nursing staff, physicians, and clinic management.
- Develop expected patient outcomes, define resource utilization, and collaborate with providers on patient goals.
- Provide nursing support and coordination to optimize patient outcomes and quality of life through clinical and educational programs.
- Support patient self-management of diseases.
- Coordinate continuity of care post-hospital admission, discharge, or ER visits.
- Manage high-risk patients, including those with multiple comorbidities or at risk for readmission, using a registry.
- Build relationships with patients and assess their physical, psychosocial, financial, and environmental needs.
- Educate patients and ancillary staff about healthcare needs, safety, and benefits.
- Utilize electronic tools like EMR for documentation and planning.
- Observe, record, and report patient conditions and reactions to treatments, and educate patients/families on procedures, medications, and health maintenance.
- Maintain and review patient records, test results, and follow-up actions.
- Respond to patient treatment-related inquiries and provide medication and treatment instructions.
- Maintain patient confidentiality.
- Document care using EMR in accordance with practice standards.
- Perform clinical and technical competencies relevant to the department.
Required Education and Experience
- High School Diploma
- Bachelor's Degree in Nursing
- At least 2 years of RN experience
Preferred Education and Experience
- Previous medical office experience
Required Licensure and Certification
- Basic Life Support (BLS) Heartsaver certification (American Heart Association) or obtained within 14 days
- Licensed Registered Nurse (RN) - Multi-State license from the State Board of Nursing
- Current active multi-state RN license or active single-state license with proof of application for multi-state license in your state of residence. Staff hired before 2/1/2024 must obtain multi-state license by 3/31/2026.
