Job summary

Mount Sinai Health System seeks a Clinical Documentation Specialist to improve outpatient documentation and coding quality in physician practices. You will review ambulatory records, educate physicians, and liaise with Health Information Management and CDQI leadership.

Qualifications

• Minimum 3 years of coding experience in a clinical setting.
• At least 1 year of HCC Risk Adjustment experience preferred.
• Credentials required: CCS, CRC, or CPC.
• Excellent communication and mentoring abilities.

Responsibilities

• Facilitate improvement in documentation and coding quality.
• Review charts to assess clinical status and history for outpatient encounters.
• Conduct coding and clinical validation audits with focus on HCC.
• Leverage billing data to identify opportunities to improve coding.
• Educate physicians and liaise between departments.
• Review CDQI alerts for accuracy.
• Ensure diagnoses and service levels reflect care delivered.
• Solicit clarification from physicians to capture key aspects of care.
• Develop performance reports and communicate results to leadership.
• Interact with physicians, particularly PCPs, in outpatient settings.
• Identify training needs and conduct coaching for clinicians.
• Monitor coding changes and educate practices on compliance.
• Ensure compliance with payer regulations.
• Perform other related duties.
