Job summary

Banner Health is seeking a Master Social Worker for the Torrington, Wyoming ambulatory care setting. You will assess patients and families, identify social and emotional needs, and collaborate with nurses and therapists to support integrated care.

Emphasis on discharge planning, resource coordination, and patient advocacy in a multi-specialty clinic context. The role requires a Master’s in Social Work with LMSW/LCSW licensure or MSW with licensure within six months; ~3 years of acute care

Qualifications

• Master’s Degree in Social Work or related field.
• LMSW or LCSW licensure or MSW with licensure within 6 months.
• 3 years acute care hospital experience preferred.
• Knowledge of Medicare/Medicaid and community resources.

Responsibilities

• Assess patients and families, identify social and emotional needs, and plan discharge.
• Develop intervention plans with the healthcare team and adjust as needed.
• Document all interventions in patient records and discharge plans.
• Provide advocacy, counseling, and crisis intervention to patients and families.
• Liaise with community resources and coordinate care post-discharge.
