Qualifications

• PharmD licensed in Rhode Island with active license.

• Ability to work collaboratively with physicians and care teams.
• Excellent written and verbal communication; maintains patient confidentiality.

Responsibilities

• Recommends pharmacotherapeutic regimens and monitoring plans to prescribers.
• Participates in disease state management and collaborates with physicians and nurse care managers.
• Monitors data and modifies pharmacotherapeutic plans as needed.
• Performs limited physical measurements (BP, glucose) for medication management.
• Documents activities in patients' medical records/EHR.
• Educates fellows, residents and students in training.
• Precepts URI IPPE/APPE students and supports education.
• Handles e-prescription refills and prior authorization submissions.
• Provides cost-effective strategies for obtaining medications.
• Provides clinical oversight to pharmacy technicians and interns.
• Other duties as assigned.
