Qualifications

• Certified BCLS.

Responsibilities

• Document all patient care activities in the EMR for accountability.
• Collaborate closely with the care team to ensure seamless, high-quality patient care.
• Collect patient data and make clinical observations that support assessments and care planning.
• Anticipate provider and patient needs through skilled nursing assessments.
• Perform pre-visit screenings for assigned patient populations.
• Assist with implementing care plan interventions and supporting patient and family education.
• Participate in quality improvement initiatives focused on outcomes, safety, and compliance.
