Job summary

Qualifications

• Graduation from an accredited practical or vocational nursing program.
• Licensed as a Practical Nurse in Missouri.
• Three years of LPN clinical experience.
• Certified BCLS.

Responsibilities

• Document all patient care activities in the EMR for accountability and reporting.
• Collaborate with the care team to ensure high-quality patient care.
• Collect patient data and make clinical observations to support assessments and care planning.
• Anticipate provider and patient needs through skilled nursing assessment.
• Perform pre-visit screenings for assigned patient populations.
• Assist with implementing care plan interventions and patient/family education.
• Participate in quality improvement initiatives focused on outcomes, safety and compliance.
• Maintain clinical competency, credentials and current standards of practice.
• Provide respectful, culturally competent care honoring patient rights and choices.
• Support a safe, inclusive environment for patients, families and staff.
• Deliver age-appropriate care addressing physical, psychosocial, educational and safety needs.
• Represent the organization with professionalism, courtesy and compassion.
• Maintain compliance with statutes, regulations, the Corporate Compliance Plan and Code of Conduct.
