Job summary

Cooper Green Mercy Health Services Authority is seeking an entry-level Clinical Pharmacist for ambulatory care in Cooper Green. Qualifications

• Active Alabama Pharmacist License
• ASHP-accredited Ambulatory Care PGY-2 residency OR ASHP-accredited PGY-1 residency with 12 months direct patient care/disease management in ambulatory care
• Current CPR/BLS certification (AHA or Red Cross)
• ASHP-Certified immunizer
• CGMSHA Credentialing process within 4 months of hire

Responsibilities

• Collaborates with providers under CPAs to manage chronic diseases, including transitions of care and diabetes education.
• Review medical records to prioritize medication needs and assess appropriateness, safety, and cost.
• Develops and implements individualized treatment plans with follow-up intervals.
• Provides patient and caregiver education on diagnosis, treatment, and safe medication use.
