Job summary

Jefferson University Physicians in Philadelphia, PA, seeks a Nurse Practitioner or Physician Assistant to join the General Cardiology ambulatory practice.

The APP will collaborate with a multidisciplinary health care team, perform H&P, order diagnostics, manage plans of care, and document progress to support high-quality patient care. Applicants should have a Master’s Degree, board certification (ACNP/PA), and BLS, and be prepared to work daytime outpatient shifts.

Qualifications

• Master's Degree
• National Board Certification as an Acute Care Nurse Practitioner or Physician Assistant
• BLS certification

Responsibilities

• Performs complete history and physicals and documents in the medical record.
• Orders and collects data using appropriate assessment techniques and diagnostic procedures.
• May serve as first responder for patient status changes, orders, and interventions.
• Assesses risks in acute and chronically ill patients, including meds, nutrition, fluids, and invasive procedures.
• Collaborates with the care team to diagnose acute and chronic conditions and manage tests.
• Documents progress, plans of care, and discharge summaries per unit standards.
• Maintains ongoing communication with the multidisciplinary team to ensure continuity of care.
