Job summary

Key Responsibilities:

• Patient Care: Provide direct nursing care to patients in their homes, ensuring the implementation of care plans in line with physicians' orders.
• Assessment & Documentation: Regularly monitor vital signs, track medical conditions, document changes, and maintain accurate health records.
• Medication Management: Monitor patients’ medication usage and report results, ensuring proper administration and dosage.
• Collaboration: Work closely with the interdisciplinary team, patients, and their families to assess, implement, and evaluate care plans.
• Personal Care: Assist with emotional, personal, and medical support for patients, providing holistic care to meet their needs.
• Care Coordination: Evaluate patient behavior, diagnose conditions, and consult with healthcare professionals to improve patient outcomes.

Working Conditions:

• Work in patient homes, often in varying environmental conditions, including possible exposure to bodily fluids and infectious diseases.
• Ability to travel locally to patient homes and manage flexible work hours.
• Occasional emergency PRN calls and exposure to adverse weather.

Qualifications:

• Education: Graduate of an accredited school of vocational nursing.
• License/Certifications: Current state LVN License; BLS Certification (required); Valid Driver’s License.
• Experience: Minimum of 2 years as an LVN in Clinical, Hospice, or Home Health settings (preferred).
