We are looking for a

Hospice Registered Nurse Case Manager / Alternate Director of Clinical Services

to help cover our newest territory in Dallas (Carrollton & Surrounding Area)

Constellation Health Services is seeking a compassionate and skilled Alt. Director of Clinical Services / Registered Nurse Case Manager to work with hospice patients and their families in homes, assisted living facilities, and skilled nursing facilities.

Responsibilities as a Registered Nurse Case Manager:

  • Provide skilled nursing care, including medication and treatment administration as prescribed.
  • Perform comprehensive assessments and coordinate care plans with all involved disciplines, families, and caregivers.
  • Monitor and report on patients' physical and emotional status, communicating changes to supervisors and physicians.
  • Participate in weekly interdisciplinary team meetings, providing updates on patient status.
  • Maintain all documentation in compliance with agency standards, utilizing electronic systems for clinical records, time, and other reports.
  • Support hospice aides and provide weekend and holiday coverage as needed.

Responsibilities as an Alternate Director of Clinical Services:

  • Oversee all clinical services provided by staff, ensuring quality and compliance.
  • Guide and support clinical staff to improve hospice care services.
  • Assist in clinical team management, planning, and evaluation of patient and family care.
  • Contribute to budget preparation and operational planning.
  • Monitor operational indicators and ensure legal compliance of clinical records.
  • Participate in performance improvement initiatives and develop organizational policies.

Qualifications

Qualifications:

  • RN licensure, education, and qualifications compliant with state regulations.
  • BSN degree from an NLN-accredited school with 3 years of full-time clinical experience, including at least one year in home health or community health.
  • Certification by the American Nurses Association as a community health nurse or related coursework in healthcare management.
  • Previous hospice/home care experience with management or supervisory roles.
  • Professional standards adherence, excellent communication skills, and understanding of hospice philosophy and pain management.
  • Valid driver's license, insured vehicle, and good driving record.
