Walnut Creek Care Community in Ohio is seeking a compassionate Licensed Practical Nurse (LPN) to provide high-quality nursing care under RNs and physicians. You will administer medications, monitor vital signs, assist with daily activities, and help develop individualized care plans.

You will document health changes, maintain accurate records, communicate with patients and families, and collaborate with the care team to ensure safe, respectful, and person-centered care in a long-term care

Qualifications

• Graduate of an accredited LPN program.
• Active LPN license is required to practice.
• Experience in healthcare or long-term care preferred.

Responsibilities

The Licensed Practical Nurse (LPN) provides high-quality nursing care under the direction of registered nurses (RNs) and physicians in a healthcare, long-term care, or assisted living setting.
