Qualifications

• Current LPN/LVN or RN or Therapist license required.
• 2+ years in long-term care required.
• Knowledge of Medicare, Medicaid, and Insurance required.
• Reliable transportation required.

Responsibilities

• Review referrals in a timely manner and provide a clinical assessment for placement.
• Collaborate with facility Admissions, Marketing Liaisons, physicians, hospital staff and case managers.
• Demonstrate knowledge of facility levels of care and network priorities.
• Read pre-admission assessments and communicate with facilities as needed.
