Job summary

River Valley Home Health and Hospice is seeking a motivated Registered Nurse to provide home health care and coordinate care plans in patients’ homes and community settings. The role emphasizes delivering quality patient care and collaborating with families to improve health outcomes.

The RN will develop and implement plans of care, educate patients and families, supervise home health aides, and collaborate with physicians to ensure the best possible outcomes.

Qualifications

• Graduated from an approved school of nursing and licensed in the state of practice.
• Minimum of two years of experience, with preference given to public health or home care nursing.
• Current CPR certification.
• Valid driver's license and insured automobile in good working order.
• Self-directed and able to work with minimal supervision.
• Excellent observation, problemsolving, and communication skills.
• Ability to organize and prioritize workload independently.
• Prolonged walking or standing, lifting, and physical agility.

Responsibilities

• Develop and initiate the plan of care in partnership with the patient, representative, and caregiver(s)
• Provide preventative and rehabilitative nursing procedures as ordered by the physician
• Offer patient, caregiver, and family counseling and education
• Prepare clinical notes and communicate with the physician and other healthcare practitioners
• Identify discharge planning needs and implement prior to patient discharge
• Communicate with the primary physician and community health related persons to coordinate the care plan
• Participate in quality assessment and performance improvement programs
• Supervise home health aides and evaluate their care

Overview:

The home health registered nurse plans, organizes, and directs/coordinates home care services. This role requires experience in nursing, with an emphasis on community health education/experience.
