Job summary

River Valley Home Health and Hospice is seeking a motivated Registered Nurse for home health services. The role focuses on planning, coordinating, and delivering high-quality in-home care, collaborating with patients, families, and care teams to meet individual needs.

The RN will develop care plans, provide preventive and rehabilitative nursing care, educate patients and families, and ensure accurate documentation while coordinating with physicians and other health professionals.

Qualifications

• Nursing degree from an approved school and current licensure in the state of practice.
• Minimum of two years of nursing experience; public health or home care preferred.
• Current CPR certification.
• Valid driver’s license and insured automobile in good working order.
• Self-directed with ability to work with minimal supervision.
• Excellent observation, problem-solving, and communication skills.
• Ability to organize and prioritize workload independently.
• Prolonged walking/standing with lifting and physical agility.

Responsibilities

• Develop and initiate the plan of care in partnership with the patient, representative, and caregiver(s).
• Provide preventative and rehabilitative nursing procedures as ordered by the physician.
• Offer patient, caregiver, and family counseling and education.
• Prepare clinical notes and communicate with the physician and other healthcare practitioners.
• Identify discharge planning needs and implement prior to patient discharge.
• Coordinate the care plan by communicating with the primary physician and community health-related persons.
• Participate in quality assessment and performance improvement programs.
• Supervise home health aides and evaluate their care.

Overview:

The home health registered nurse plans, organizes, and directs/coordinates home care services. This role requires experience in nursing, with an emphasis on community health education/experience. As a professional nurse, you will utilize the resources of the community to plan and direct services that meet the needs of individuals and families within their homes and communities.
