• Degree from an accredited Physical Therapy Program
• Current Professional licensure as a Physical Therapist
• Current CPR Certification

Responsibilities

• Provide initial evaluation and establish a treatment plan with functional/work goals
• Progress treatment and document progress
• Communicate with physicians in writing in a timely manner
• Manage discharge planning and follow-up with patients
• Supervise rehabilitation aides and PTAs as applicable
• Document treatment and communications in the chart
• Build professional relationships with referral sources including physicians and case managers
