Job summary

Adaptive Home Health is seeking a Registered Nurse (RN) Case Manager for field-based, full-time work in Detroit area. You will conduct in-home assessments, document OASIS/SOC/recert, and manage high-acuity patient care including wound care.

Lead care plans with physicians and supervise LPNs and HHAs, while building strong relationships with referrals. Join a team aiming to deliver exceptional patient care through AI-native tools, optimized routing, and streamlined charting to reduce after-hours

Qualifications

• Active Michigan RN license required.
• Home health / OASIS experience strongly preferred.
• Ability to manage complex patient plans and collaborate with clinicians.

Responsibilities

• Perform comprehensive in-home assessments and maintain skilled documentation (OASIS, SOC/recert/ROC, discharge).
• Provide direct, high-acuity patient care with focus on wound care.
• Develop and manage the plan of care with physicians; supervise LPNs and HHAs on your caseload.
• Represent Adaptive to patients, families, and referral sources.
