Zusammenfassung

DOROT, Inc. is seeking a Synagogue Partnership Social Worker to support older adults and caregivers in Manhattan’s Jewish community. The role requires collaboration with nine synagogues, case management, and program development to reduce social isolation.

You will work in a hybrid schedule, joining colleagues in the office and conducting outreach via home visits and outreach calls. Salary ranges reflect NYC standards and generous benefits.

Qualifikationen

• LMSW or Licensed Mental Health Clinician required.
• 4+ years of professional relevant work experience preferred.
• Deep knowledge of issues affecting older adults and caregivers, NYC area preferred.
• Experience collaborating with clergy, staff, and volunteers in community settings.

Aufgaben

• Be a vital member of a two-person team serving 250+ older adults yearly with one-time and ongoing interactions.
• Conduct outreach calls and home visits; provide case assistance, counseling, and referrals.
• Liaise with nine synagogue partners; promote awareness and program opportunities.
• Maintain at least 20 Friendly Visiting matches; support aging-related needs.
• Collaborate with rabbis and synagogue leaders on aging and caregiver issues.
• Stay informed on aging-related topics and community resources.
• Assist with presentations on aging and social isolation for synagogue communities.
• Lead or co-lead support groups on isolation, bereavement, caregiving.
• Support synagogue care committees and lay leadership.
• Enter and maintain case data in Salesforce; perform timely notes and entries.
• Provide Information & Referral coverage as needed.
• Represent DOROT at events; collaborate with departments to promote programs.
